How pain is understood
Pain after exertion or prolonged posture may change with movement and rest. When it spreads into the arm or leg with numbness and weakness, nerve involvement is considered. Prolonged morning stiffness and pain that improves with activity may call for assessment for an inflammatory disease.
In pain that persists, poor sleep, stress, fear of movement and reduced activity can make recovery harder. This does not make the pain unreal. These factors are assessed alongside physical causes and can influence how rehabilitation is organized.
When physiotherapy should be preceded by urgent assessment
Changes in gait and in hand coordination, fever, a history of cancer or unexplained weight loss require a prompt medical consultation. A massage session should not delay this assessment.
What the functional assessment includes
The physiotherapist asks which activities are limited and which movements increase or ease the symptoms. They assess mobility, strength, endurance and the way you carry out ordinary tasks. When there are nerve signs or suspicion of another condition, a consultation with the relevant doctor is needed.
A report with degenerative changes does not by itself determine the program. Imaging findings are interpreted together with symptoms and examination. In ordinary pain without warning signs, repeated tests are not a precondition for starting active care.
How the program is built
The program may combine controlled movement, strengthening, endurance exercises and aerobic activity. The choice depends on starting capacity, preferences and the response to load. Goals should be concrete, such as walking farther, sitting with fewer interruptions or returning to work.
Manual techniques may be used for symptom relief when appropriate, but long-term function also requires regular activity. The program should not rely only on the treatments you receive in the session. Home exercises and their adaptation are part of the plan.
Posture at work and activity at home
Change position during the day and break long periods of sitting with short movements. Adjust the screen and workstation so that you do not need to bend the neck continuously. Holding the body rigid in a single position all day is not required.
Continue tolerable tasks and avoid prolonged bed rest. In phases with more pain, the load may be reduced temporarily and rebuilt. Supports such as a collar are not used routinely without an indication. Local heat or cold may help temporarily, with skin protection.
Frequently asked questions
How many physiotherapy sessions will I need? The number depends on the cause, the duration of the pain and your goals. The plan is based not only on sessions but also on regular home exercises. Progress is reviewed after a few sessions and, if the expected improvement does not occur, the diagnosis and program are reassessed.
Do I need an MRI before starting physiotherapy? In ordinary pain without warning signs it is not necessary. Imaging is requested when there is weakness, spreading numbness, suspicion of another cause or when the result will change treatment. Degenerative findings are common also in people without pain.
Is it harmful to exercise when it hurts? Mild, temporary discomfort while adapting is usually acceptable and may call for lowering the intensity. Severe pain, new numbness or loss of strength require stopping and assessment. The physiotherapist should explain the difference.
Is a neck collar useful for neck pain? Not routinely. Prolonged use of a collar may weaken movement and muscles and is not recommended without an indication. Tolerable movement, exercises and adjustment of the workstation are usually preferred.
How progress is assessed
A single session does not determine the course. Changes in sleep, tolerance of activity and the ability to do the target tasks are followed. If pain keeps worsening, weakness appears or the expected progress is not achieved, the diagnosis and the plan are reviewed.
Mild discomfort while adapting to exercise may call for a change of intensity; severe pain, new numbness or loss of strength require stopping and assessment. The physiotherapist should explain how to tell an acceptable reaction from one that needs help.
Care at Tirana Polyclinic: We offer physiotherapy coordinated with orthopedics, neurology, neurosurgery or rheumatology depending on the problem. We interpret imaging performed at other centers and adapt the plan to your condition and goals.
